Job Description

Provides initial contact for consumers seeking medication services at Hudson. Obtains information to prepare for medications services. Works as part of a treatment team to insure needed services are provided. Creates a welcoming and engaging environment.

DUTIES AND RESPONSIBILITIES (including but not limited to):


Takes vitals to include blood pressure, pulse, weight, height and BMI on all consumers presenting for a medication clinic appointment.

Obtains the chief complaint or presenting problem for seeking medication services.

Updates information that includes but is not limited to any hospitalizations, new medications, changes in health status since  last clinic visit.

Reviews medications to determine if medications are being taken as prescribed and if any medications are not beings taken.

Faxes and/or contacts the appropriate pharmacy and supplies them with physician orders.  Under physicians’ supervision, distributes sample medications and patient assistance prescription coupon vouchers to eligible consumers.

Reviews and documents all refill requests from consumers.  Researches refill requests and obtains physician approval if consumer is refill eligible.  Prepares refills for pickup or contacts the appropriate pharmacy.  Contacts consumers who are not refill eligible and assists them in scheduling a return appointment with the physician.

Monitors and responds when appropriate to medication/refill voice mail line

Maintains contact with pharmaceutical representatives in order to maintain sample supply

Monitors attendance and follows up on missed appointments

Provides accurate and timely documentation in the consumer’s chart and OrderConnect e-prescribing software

Conducts screenings and upfront CM services as appropriate for walk in consumers with no assigned clinical team

Completes discharges as requested for clients no longer engaging in services

Provides services that are welcoming, engaging and accessible.  These services are recovery focused, strengths based, culturally competent, consumer centered, and co-occurring capable.  Works as part of treatment team to meet the needs of the consumer in a timely and thorough manner.  Interacts with consumers in a respectful, genuine and professional manner. 

Participates in trainings/workshops related to triage, assessment, crisis intervention, case management and in expanding knowledge base in recovery focused mental health services and substance abuse services.  Completes all required courses with NetSmart University.

Maintains professional competency in the area under his/her responsibility through appropriate readings, attendance at workshops and conferences, and through consultation with other professionals. Employee will comply with all requirements to maintain professional certification.

Performs other duties as required and/or assigned by Supervisor

Must maintain 25% productivity expectation

Skills & Requirements

Candidate/employee must possess a high school diploma or GED. A bachelor’s degree in psychology social work or other related human services field, Case Management certification or Certification as a Recovery Support Specialist is preferred.

Must demonstrate good interpersonal skills and the ability to effectively communicate verbally with English-speaking consumers.  Must display proficiency in the MS Word processing software and intermediate general computer skills.   Local travel is required. Must possess and provide verification of current automobile liability insurance including uninsured motorist coverage. Employee must be able to move about the facility freely and be able to sit for extended periods.  Must have current (within last year) TB test, or obtain within first 30 days of employment.  Must possess and maintain current certification in CPR, First Aid, and CAPE; or obtain within 30 days of employment with NorthCare. Employee must possess or obtain Core Level Co-Occurring Training and Trauma Informed Training within 90 days of employment with NorthCare.
