Due to anticipated organizational growth, Liberty Healthcare is hiring for several new full-time Area Supervisor positions.

As an Area Supervisor, you will support people who desire to transition from long-term care facilities into their homes or other community settings.

You will help people live where they want to live, ensure they receive quality healthcare and human service supports, and achieve positive health outcomes.

Your daily work will involve:

  • Supervising Team Coordinators who work directly with clients transitioning out of long-term care
  • Quality assurance oversight of client assessments and transition plans
  • Coordinating home and community-based services
  • Serving as the primary point of contact with advocates and decision makers
  • Participating in meetings and case conferences

This role will allow you to work from home partially and require you to travel throughout the region regularly.

Skills & Requirements

You are a good fit for this position if you possess the following:

  • Bachelor’s degree in Social Work, Nursing, or other healthcare / human services field
  • At least three years of experience in home and community based services or long term care
  • Assessment or MDS experience involving person centered planning and identifying beneficiary needs for a safe living environment
  • Experience supporting and deploying community resources to older adults or people who are living with physical disabilities or intellectual and developmental disabilities
  • Supervisory, management or other leadership experience
  • Knowledge of healthcare assessments, case management, and quality assurance methodologies
  • Willingness and ability to travel throughout the region
  • Good organizational skills and time management skills
  • Ability to effectively communicate with a diverse work force and beneficiary population
