Case Manager: The Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

  • Initial and ongoing comprehensive assessments (OASIS).
  • Develops a plan of care with clinical team.
  • Perform nursing care according to the needs of the patient.
  • Have a good understand of Medicare guidelines.

Skills & Requirements

Qualifications and Experience

  • Graduate of an accredited school of nursing.
  • Currently licensed.
  • One (1) year nursing experience recommended, experience in home health care recommended.
  • Must have and maintain a reliable automobile / auto insurance.
  • A criminal background check and drug screen must be performed before hire.
