Essential Duties and Responsibilities *
The purpose of the Care Coordination Representative is to assist the RN Case Manager with daily case/client management tasks, records management, process monitoring for effective clinical and non-clinical outcomes
Ensure appropriate clinical and client signatures, and dates on all paperwork/electronic forms.
Verify required completed documentation for comprehensive assessments at start of care, recertification, resumption of care, transfers, and discharges prior to scanning.
Work with field staff to correct order inaccuracies
Knowledge of agency policies, regulations, and procedures
Prepares, reviews and cleans-up organization monitoring reports
