The New Jewish Home, Sarah Neuman campus in Westchester, is currently seeking Full Time Day Shift Dementia Coordinator to join their team!

Job Summary: The successful candidates will be reporting to the Assistant Director of Nursing and will be responsible for the coordination and ensuring the quality of all nursing care during his/her assigned shift. 

Job Duties:

• Supervises the delivery of nursing care within assigned unit
• Effectively communicates resident/unit needs to Nursing Administration
• Responsible for assuring the resident care plan is revised as necessary to reflect current needs

Job Requirements:

• Valid NYS RN Professional License
• BSN preferred
• Minimum of four years experience with some of those in an acute care setting as well as a charge nurse role
• Must be certified or training in process
• Availability to work assigned 7am - 3pm shift, every other weekend
