Job Description

Coordinates a patient focused team in providing direct and collaborative health care in a variety of clinical settings designed to promote and restore optimal health across the life span. Utilizes all components of the nursing process in providing care that reflects an understanding of individual cultrural and religious diversities and needs, and recognition of patient rights. Ultilizes knowledge and expertise in case management to provide comprehensive, coordinated, seamless care to inviduals and aggregate groups. Coordinates, intergrates and facilitates the patient/ provider relationship along the continuum of care. 

Responsibilities include, but are not limited to:

  • Accurately completes the Daily Patient Care Flow Sheet.  Writes notes that clearly reflect the patient’s condition including daily progress notes, and discharge and or transfer notes. Documentation is accurate, concise and relevant and includes the patient’s response to interventions.
  • Safely administers medication on time in accordance with the Hospital’s policies.  Is knowledgeable about medications including purpose, side effects, and contraindications.  Can educate the patient appropriately.
  • Is familiar with Hospital Policies and Procedures and knows where to access them if needed.  Uses charting forms appropriately.
  • Is familiar with regulatory requirements and best practice and complies with both in the delivery of care.
  •  Interacts collaboratively with physicians and members of other departments to insure patient care needs are met.
  • Inform physicians promptly of changes in the patient’s condition.
  • Provides an accurate report to the Nurse Manager or designee and the oncoming nurse each shift.  Performs as a Charge Nurse when assigned and demonstrates leadership

 





Skills/Requirements
  • Valid NYS RN License
  • BSN preferred
  • One year practice in a medical/surgical setting.
  • American Heart Association certification in Basic Life Support (BCLS)
