Job Description

Job Summary

The Wound /Ostomy Coordinator is responsible for providing Wound and Ostomy care to our patients. In this role, you will function as a resource person to physicians and management in matters regarding the provision of care. Further, you will recommend appropriate wound care/ostomy management strategies by collaborating with Physicians and staff, patients and their families.

Primary responsibilities include, but are not limited to:

  1. Communicates with physicians and case managers regarding clinical findings and recommendations
  2. Perform clinical assessments of patients and create care plans
  3. Formulate policies and education relative to wound care/ostomy management
  4. Develops programs to enhance service provided to wound care/ostomy patients.
  5. Performs clinical intervention skills based on identified client problems and hospital protocols for in patients and out patients.
  6. Provides direct specialized care to individuals and families that is effective and individualized for the specific patient situation
  7. Able to demonstrate working knowledge of current ostomy and wound care therapy including negative pressure wound therapy.

 

 





Skills/Requirements

Requirements

  • Valid NYS Registered NP License
  • Wound and Ostomy Certification Required
  • A minimum of 2 years Medical Surgical Nursing /Wound Ostomy Care
  • Strong clinical assessment skills
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
