Job Description

Liberty seeks a Registered Nurse for an available PRN “Independent Assessor” position. This is a field-based position which allows for some work from home and also requires regular travel to client homes throughout Davidson County and Davie County. As an Independent Assessor with Liberty, your role would be to assess clients to help determine their eligibility for Medicaid-funded personal care services. This would include: Working on an as needed basis Traveling throughout the local region Visiting clients in their homes Assessing client’s activities of daily living and evaluating client’s home environment Completing electronic reports related to client’s health status, diagnoses, medications and other pertinent factors As a PRN employee of Liberty you can expect: Competitive reimbursement for each completed assessment Mileage reimbursement Monday through Friday work schedule (no weekends) PRN employees are given consideration for future openings for part-time and full-time positions.
Skills/Requirements
