Job Description:                  Registered Nurse/Skin Care

Summary:  Flagler Hospital’s Skincare Team is a unique group of nurses within the overall team of the Rehab Department, which also includes PT, OT, and ST.  Our primary role is to serve as consultants for pressure ulcer prevention and wound care and also involved with patient teaching for ostomies.  Our nurses must have independent working skills as well as a continually growing knowledge base in wound therapies.  Our services are available to the patients 7 days/ week from 8:30-4:30pm.  We interact with all disciplines, and work closely with nurses and physicians to provide our services to all patients in all areas of the hospital.

Responsible To:                   Director of Rehab Services


Education/Training:             Current Florida nursing licensure required; BSN preferred.


Experience:                            RN with one year Medical Surgical or Home Health Care experience required. Wound care experience and certification preferred but not required.   


Skills & Abilities:                 Strong nursing assessment skills. Demonstrated knowledge and skills necessary to provide care to and communicate with primarily geriatric population and to a lesser degree pediatric and adult.  Understands the goals and objectives of skin care team; Posses and utilize positive communication skills between disciplines, departments and medical staff.  Willingness to foster a team/player/customer orientated philosophy among all members of health care team.  Possess strong teaching skills, creating educational newsletters for staff. 


Physical Requirements:   Strong physical demands.  Constant pulling, pushing, stooping, lifting, walking and occasional sitting and computer work


Environmental Conditions:   Mainly working inside.


Occupational Exposure:      Category:  I = Job involves tasks or procedures that have exposure to blood and potentially infectious materials during the performance of duties.





  1. Provides wound and ostomy assessment via nursing assessment triggers or physician orders.  Wound care assessment is completed within 24 hours of referral.  
  2. Evaluation, reassessment, treatment and education are based on patient age and specific needs particularly in the areas of body system patient, functional, psychological function and nutritional status.
  3. Discuss with physician, staff, patient and family on the appropriateness or inappropriateness of referral.
  4. Develops a treatment plan of care for the prevention of breakdown and or for those patients with ulcers. Determines level of risk.  Discusses and documents treatment approach to staff nurse; Develops treatment plan of care for ostomy management during hospitalization, including access to supplies and appliance changes as needed.
  5. Reassess patient as indicated by protocols documents all checks, assessments, changes to the plan of care in patient chart.  Notify staff of changes and physician when necessary. 
  6. Orders appropriate pressure relief systems and ostomy appliances that have been approved for use at Flagler Hospital.  Makes referrals to other disciplines as indicated by the needs of the patient.  Provides patient/family education on wound care and ostomy management.   Documents same on Multi-Disciplinary Education Form; Provides education to nursing, dietary and rehab staff on new wound care and ostomy products, and or changes in wound care program, or ostomy management.
  7. Assists with discharge planning for continued decubitus management and ostomy education for new ostomy patients as needed.  Assist with materials management requests regarding appropriate ostomy and wound care supplies for in hospital stock.  Reports to wound care ad hoc team development/expansion/ changes to wound care program and ostomy management and follows recommendations made by the ad hoc committee            
  8. Participates in hospital’s quality improvement program.
  9. Performs other job related duties as assigned by Director.
