Job Description:                                 PHARMACIST (STAFF)


Department:                                         Pharmacy


Responsible To:                                   Director of Pharmacy


Education/Training:                            Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) or graduate of a foreign college of pharmacy; currently licensed to practice pharmacy in the State of Florida and BLS required.


Experience:                                           One year previous experience in hospital pharmacy desirable.                 


Skills & Abilities:                  Good physical and mental health; must be willing to work with the realization that errors may have serious consequences to patients.  Must have a memory for details and be alert to detect errors in compounding.  Must be able to cooperate with other employees and have the ability to supervise subordinate workers.  Must have ability to utilize all necessary reference book and textbooks related to medicine and pharmacy.


Physical Requirements:                      The physical activities include walking, stairs climbing, reaching to obtain stock, and the picking up/toting/putting down of heavy items.


Environmental Conditions:               Potential for exposure to infectious patients or materials; potential for exposure to hazardous and toxic substances (including chemotherapy, cytotoxic drugs, and cleaning solutions);  potential for sticks or cuts by needles and other sharp items.  Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.


Occupational Exposure:                    Category III = Job involves tasks or procedures that have exposure to blood and other potentially infectious materials during the performance of duties.



  1. Accurately interprets, profiles and fills physicians’ orders.
  2. Maintains patient medication records via computerized programs.
  3. Monitors drug therapy for duplicates, contraindications, interactions, incompatibilities, allergies, and dosing errors.  Documents interventions as needed.  Monitors Aminoglycosides and vancomycin therapy utilizing pharmacokinetic principles.
  4. Intervenes with physician to clarify medication orders, dosage, frequency, and route as needed.  Documents changes in Medical Record as per telephone order.
  5. Dispenses control drugs and maintains accurate records and inventory as required by law and pharmacy policies. Maintains inventory of pharmaceuticals and supplies, and ensures the proper storage condition, removing all medications past their expiration date and/or no longer suitable to be in inventory. Participates in stock control programs (ordering, inventorying, and monitoring usage) as required.
  6. Obtains emergency drug items or out of stock items as needed per pharmacy policy and reviews all non-formulary requests for formulary alternatives, prior to dispensing.
  7. Ensures correct placement of medication in individual cassettes for in-patients, either verifying placement by supportive persons or filling cassettes.
  8. Aseptically prepares intravenous fluids, irrigation solutions, additives, chemotherapy and other sterile products. Provides drug information to the hospital’s professional staff.
  9. Provides drug information to the hospital’s professional staff.
  10. Participates in pharmacy orientation programs, pharmacy staff meetings, and in-service educational programs. Attends and participates in other programs, committees, meetings, and functions required by the hospital or the pharmacy. Participates in training new staff members or volunteers as needed. Participates in routine back-ups of the computer programs, libraries and databases. Participates in quality assurance programs.
  11. Performs other job related duties as assigned by Director.
