Job Description | ||||||||||||||||||||||
-Develop a comprehensive nutrition assessment and care plan for patients /residents. -Document subjective and objective information, assessment, and intervention in Electronic Medical Record. -Develop an individualized care plan based on the specific needs of each resident / patient. - Communicate with the interdisciplinary team regarding nutrition care and goals. - Review and revise care plans as necessary, but at least quarterly. - Attend and participate in the Interdisciplinary Care Planning Meetings, pertinent staff meetings, and other area meetings as applicable. - Utilize Best Practice guidelines to document on the nutritional progress of each resident/ patient. -Plan, calculate, and modify therapuetic diets, tube feedings etc. for residents as prescribed by the physician. -Review all diets and menus to assure compliance with MD orders; make recommendations as needed. - Conduct regular meal rounds. -Maintain updated and accurate records. -Maintain confidentiality of patient / resident clinical information. -Comply with organization policies and procedures. |

Skills/Requirements | ||||||||||||||||||||||
Minimum of (1) year of professional work experience as a Registered Clinical Dietitian in a long term care or similiar healthcare setting preferred. Will consider recent grad from an accredited college or university, who is R.D. eligible -Organized and possess excellent written and oral communication skills. Able to perform responsibilities with minimum supervision Basic computer skills required. |
