Liberty Healthcare Corporation is seeking a Case Manager for The Westchester Behavioral Health Center. This professional clinical position is responsible for providing case management and discharge planning services for adult patients in the inpatient unit. The Case Manager will be a key member of the treatment team and its process.

Primary Duties and Responsibilities:
•Provide clinical support and backup to other clinical staff as needed.
•Actively participate in multidisciplinary team processes, including individualized treatment planning, case reviews, behavioral reviews and discharge planning.
•Work with relatives and others concerned in making psychiatric treatment plans effective and interacts with community agencies involved in patient’s care and discharge procedures.
•Contribute in staff meetings, case conferences, case presentations, and training activities.
•Provide training and consultation to direct care staff on therapeutic and milieu methods and procedures.
•Prepares patient records and required treatment records and maintains required documentation of work activities.
•Perform administrative duties as a member of the treatment team including discipline specific reports, attending staff meetings, and providing technical assistance.
Minimum, Knowledge, Skills, and Ability Requirements:
•The incumbent must have knowledge of crisis intervention principles, practices and case management.
•Knowledge of medical and pharmacology terminology.
•Skills to detect unusual patient behaviors through observing people who are at risk because of health problems, drug abuse or other health related problems.
•Good writing skills to document patient’s condition, treatment and care plan in records.
•Establish and maintain effective interpersonal relationships with patients, co-workers, supervisors, physicians and the general public and work in a treatment team environment.
•Knowledge of Joint Commission standards preferred.
•Commitment to enhancing cultural competency.
