Job Description:                                  PHYSICAL THERAPIST


Department:                                          Rehab Services


Responsible To:                                   Director of Rehab Services


Education/Training:                             Graduate of an APTA accredited Physical Therapy program; licensure in the State of Florida required prior to start date.  BLS required.


Experience:                                            One-year experience preferred


Skills & Abilities:                Demonstrated knowledge and skills necessary to provide care to and communicate with primarily geriatric population and to a lesser degree pediatric and adult.  Excellent communication skills; team player.  Ability to customize treatment plan for each patient based on their medical, physical and age related needs.  Provide excellent customer service.


Physical Requirements:                       Strong physical demands: Constant pulling, pushing, stooping, lifting, walking and occasional sitting.


Environmental Conditions:                 Mainly works inside. 


Occupational Exposure:                      Category I= Job involves tasks or procedures that have exposure to blood and potentially infectious materials during the performance of duties.






  1.  Evaluation, reassessment, treatment and education are developed based on patients age specific needs particularly in the areas of body system function, musculoskeletal function, muscle strengthening and flexibility, improve endurance, pain management, and overall performance of functional activities, psychological function, nutritional status, and educational needs.
  2. Reassesses the rehabilitation needs of each patient.  Establishes realistic problem list and attainable goals appropriate to the setting.  Makes necessary revisions to the plan of care, adjusting problems and goals as indicated.
  3. Ongoing treatment of patient is focused on relieving pain, developing and/or restoring function and maintaining maximum performance utilizing appropriate physical therapy procedures and testing. 
  4. Educates and documents in the patient record instructions given to; patient, caregiver, family member or other health care personnel.  Instructions prepare patient and family to follow through with treatment programs at home.  Completes all documentation and patient care with accuracy and efficiency and within rehab guidelines.
  5. Recommends referrals to other disciplines and confers with other members of health care team to develop smooth, coordinated delivery of services, including discharge planning.  Meets the productivity standards that have been set by management. Utilizes the rehab aides efficiently and effectively.
  6. Demonstrates an ability to be resourceful “think on your feet” delegate patient caseload to other staff as needed.  Demonstrates a strong understanding of ICD 9 codes, CPT codes and their definitions, CCI edits.
  7. Utilizes guide to physical therapy practice to aid in treatment planning, progression and discharge.
  8. Requires minimal supervision and is self directed. Demonstrates a strong, positive self image of own skills, judgment and capability. Considers effects of words and actions on others.  Works towards resolution of interpersonal conflicts as they arise.  Maintains hospital property in a manner that demonstrates ownership and responsibility.
  9. Performs other job related duties as assigned by Director.
