Job Description:                                   Discharge Planner                                            

               

Department:                                         Social Services                                                                                    

 

Responsible To:                                   Director of Case Management                                                        

 

Licensure:                                             N/A

 

Education/Training:                            Bachelors Degree in Social Work preferred. Equivalent degree with experience in the medical field and community resource knowledge considered.

 

Experience:                                           Prefer experience in an acute hospital setting or direct discharge planning from a medical setting.  Strong relationships with community resources and skills to maintain rapport with community based organizations a mustBasic computer skills needed.     

 

Skills & Abilities:                                  Self motivated and highly organized with the ability to multi task.  Able to initiate discharge planning and engage patient/family/caregiver and interdisciplinary medical treatment team as appropriate. Act as patient advocate to help resolve concerns, complaints, and requests. Assure patients progression through the continuum of care and arrange discharge to the least restrictive environment.  Customer service oriented with emphasis on active listening and critical thinking skills.   Ability to work with minimal supervision and meet imposed deadlines.

               

Physical Requirements:                      Constant walking and standing. Some bending and stooping required.  Light amounts of the following: reaching above/below the waist; fine hand/finger movements; excellent visual acuity to read and screen medical records; excellent listening and auditory skills to work over the telephone and in person.

 

Environmental Conditions:               Primarily inside (> 75% of time) with occasional exposure to extreme weather conditions.

 

Occupational Exposure:                    Category II = Job involves tasks or procedures that have some exposure to blood and other potentially infectious materials during the performance of duties.                 

Skills/Requirements

ESSENTIAL RESPONSIBILITIES:

  1. Assures that social work services are readily available to the patient, the patient’s family and other persons significant to the patient.
  2. Responds to consults in a timely manner according to policy.
  3. Assertive in initiating involvement with patient/family/caregiver and treatment team as appropriate to formulate a discharge plan.
  4. Multidisciplinary engagement in the discharge planning process.
  5. Documentation of actions to include options presented as mandated by MCR and JACHO to patient and/or family.
  6. Acts as patient advocate to help resolve concerns, complaints, and requests.
  7. Submits timely and accurate reports as requested and displays organization on face sheets with updated information, follow list and contact information
  8. Works closely with all members of Medical Staff to assure positive physician working relationships.
  9. Actively reports patient complaints/concerns to respective department director and coordinates corrective action as indicated.
  10. Demonstrates the knowledge and skills necessary to provide care, based on the physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients(s) served.

11.    When possible documents in real time with clear and concise language.

12.    Performs other job related duties as assigned by the Director.
