Job Description:                                 Data Management/Diabetes Community Liaison                    

               

Department:                                         Disease Management                                                        

 

Responsible To:                                   Director                                                                                 

 

Special Requirements:                      Position requires frequent local travel to various off-campus community settings and work at the hospital. The candidate must provide his or her own transportation (subject to mileage reimbursement) and must have valid Florida Drivers License; must have and maintain proof of Florida Motor Vehicle Insurance as an ongoing condition of employment; and must have safe driving history for at least 24 months. 

 

Education/Training:                            High school or equivalent required.

 

Experience:                                           Computer experience required for data entry and generating reports.  LPN or C.N.A. experience desired. Background in community service/public health preferred.

 

Skills & Abilities:                  Self motivated, excellent communication skills; Bilingual (Spanish) desirable but not required.

 

Physical Requirements:                      Must possess the ability to handle difficult situations with emotional stability; ability to communicate well (seeing and hearing); manual dexterity to type, answer telephones and file records.

               

Environmental Conditions:               Local travel required to work in various community settings and at the hospital. 

 

Occupational Exposure:                    Category II = Job involves tasks or procedures that have some exposure to blood and other potentially infectious materials during the performance of duties.                               

                               

Skills/Requirements

ESSENTIAL RESPONSIBILITIES:

  1. Collect and enter data into the computer for follow up and quality assurance.
  2. Assist diabetes resource nurse in data collection/entry for the Joint Commission advanced certification for inpatient diabetes program.
  3. Assist diabetes resource nurse in coordinating community health initiatives throughout Saint John’s County, particularly in underserved populations.
  4. Participate in health fairs, community events, employee wellness activities as directed by diabetes resource nurse, disease management director.
  5. Provide direct education in person and via telephone both to the community and hospitalized patients related to diabetes and other health conditions.
  6. Provide direct health screening, including, but not limited to, blood pressure and fingerstick glucose.
  7. Assist in projects as assigned to achieve/maintain the Joint Commission advanced certification for inpatient diabetes program.
  8. Perform other job related duties and projects as assigned by Disease Management director/ Chief Medical Officer.
