Job Description:                                   Occupational Therapist

 

Department:                                         Rehab Services

 

Responsible To:                                   Director of Rehab Services

 

Education/Training:                            Graduate of an AOTA accredited Occupational Therapy program, licensure in the State of Florida

 

Experience:                                           Prefer one year’s experience in acute care and/or SNF care.

 

Skills & Abilities:                                  Demonstrated knowledge and skills necessary to provide care to and communicate with primarily geriatric population and to a lesser degree pediatric and adult.  Excellent communication skills/team player.  Ability to customize treatment plan for each patient based on their medical, physical and age related needs.  Provide excellent customer service.

 

Physical Requirements:                      Strong physical demands. Constant pulling, pushing, stooping, lifting, walking and occasional sitting

 

Environmental Conditions:               Mainly working inside.

 

Occupational Exposure:                    Category I= Job involves tasks or procedures that have exposure to blood and potentially infectious materials during the performance of duties.

 

Skills/Requirements

 

ESSENTIAL RESPONSIBILITIES

 

  1. Evaluation, reassessment, treatment and education are developed based on patients age specific needs particularly in the areas of body system function, cognition, psychological function, nutritional status, overall performance of ADL’s, IADL’s and educational needs.
  2. Reassesses the rehabilitation needs of each patient.  Establishes realistic problem list and attainable goals appropriate to the setting.  Makes necessary revisions to the plan of care, adjusting problems and goals as indicated.
  3. Ongoing treatment of patient is focused on relieving pain, developing and/or  restoring function  and maintaining maximum performance of all ADL’s, IADL’s utilizing appropriate occupational therapy procedures and testing systems.
  4. Educates patient, family, caregiver and personnel on alternate techniques and use of adaptive equipment to complete ADL’s.  Educates and documents in the patient record instructions given to; patient, caregiver, family member or other health care personnel.  Instructions prepare patient and family to follow through with treatment programs at home.
  5. Completes all documentation and patient care with accuracy and efficiency and within rehab guidelines.
  6. Recommends referrals to other disciplines and confers with other members of health care teamto develop smooth, coordinated delivery of services, including discharge planning.
  7. Meets the productivity standards that have been set by management.
  8. Utilizes the rehab aides efficiently and effectively.
  9. Demonstrates a strong understanding of ICD 9 codes, CPT codes and CCI edits.
  10. Demonstrates an ability to be resourceful “think on your feet” delegate patient caseload to other staff as needed.  Requires minimal supervision and is self directed.
  11. Performs other job related duties as assigned by Director.
