Job Description

Liberty Healthcare seeks an experienced Social Worker or Community Services Director with strong analytical skills for an available full-time Regional Coordinator opportunity.

Field-based position covering 17 northwestern PA counties: Erie, Crawford, Mercer, Lawrence, Butler, Venango, Armstrong, Clarion, Forest, Warren, Jefferson, Elk, McKean, Clearfield, Cameron, Potter, Centre

The mission of the PA Link Network is to improve access to long-term care supports through an integrated network of partners committed to expanding the use of community-based solutions, promoting consumer directed decision-making, and enhancing efficiency and quality of service.

Aging and Disability Resource Centers (ADRC) are a nationwide effort to take a seamless approach in the way States assist seniors and adults with disabilities who need help with activities of daily living. The ADRC in Pennsylvania is known as the Link.

The PA Link Network\ADRC Regional Coordinator will provide start-up and ongoing technical assistance to County Link Networks within a 17 county northwestern Pennsylvania region.

This is a field based position. Significant time (2-4 days / week average) will be spent traveling within this region. All travel (including mileage) is reimbursed. Computer skills are essential. Laptop, mobile phone and mobile internet connection provided.

Primary Responsibilities:

  • Under the direction of the Project Manager, manage day-to-day operational aspects
    • Effectively communicate project expectations and progress to project team in a timely and clear fashion
    • Identify and resolve issues and conflicts within the coordinating offices
    • Develop, analyze, and troubleshoot project schedules
  • Coordinate internal and external meetings (face-to-face and conference calls)
  • Provide oral and written updates on project status to a variety of audience levels
  • Public speaking 
  • Up to 80% travel within designated area
  • Assists with development, standardization, implementation and maintenance of project  policies and procedures as well as current or potential grant opportunities
  • Monitors program objectives through review, evaluation, and analysis of program and budgetary data
  • Responds to inquires\questions related to project in designated region

Selected professional staff at the PA Link Network work through a collaborative arrangement and contract with Liberty Healthcare Corporation.

As a full-time employee of Liberty Healthcare serving this role you can expect:

  • Compensation: $25.64 per hour (approximately $49,998 per year)
  • $2,600 additional compensation annually for those employees who do not require health insurance
  • 25 days of paid time off annually for holidays, vacation, sick and personal leave
  • 37.5 hour workweek including travel time; No nights or weekends

Skills/Requirements
  • Bachelor’s degree in social work, human services or related field
  • Knowledge and experience in home and community based services for persons over the age of sixty and adults with disabilities
  • Strong communication ability; both orally and in writing
  • The ability to build, develop, and grow business relationships vital to the success of the project
  • Strong organizational skills with demonstrated ability to work independently under general supervision
  • The ability to use Microsoft Office Suite and other program-specific data collection systems
  • The ability to develop comprehensive reports; prepare and make formal presentations
