Job Description:                                 Social Worker/Counselor

                                                                Behavioral Health   (Clinical and/or Crisis)

 

Department:                                         Behavioral Health Unit

 

Responsible To:                                   Director of Behavioral Health

 

Education/Training ED:                      Master’s Degree in Clinical Social Work, Marriage and Family Therapy, Mental Health Counseling, or other Mental Health related Master’s Degree program that is License eligible by the State of Florida.  Must successfully complete NAPPI training (Non-Abusive Psychological & Physical Intervention) within 90-days of employment; annual recertification will be required thereafter.

 

License:                                                 Florida license in Clinical Social Work, Marriage and Family Therapy, or Mental Health Counseling required.  BLS required.

 

Experience:                                           Minimum of three (3) years mental health experience in the clinical responsibilities listed below. Job duties may be performed in the Flagler Hospital Emergency Department (FHED) and/or the Behavioral Health Unit.

 

Physical Requirements:                      Requires stooping, kneeling, reaching, handling, feeling, talking, hearing, and frequent mental/visual strain.  Occasional heavy work (exerting 50-100 lbs. of force up to 1/3 of the time); frequently exerting moderate force (21-50lbs of force up to 2/3 of the time); and constantly exerting force (10-20 lbs. of force) to move objects and patients.

 

Environmental Conditions:               Primarily works inside (75% of time) but may experience the following hazards: biological, radiation and stress.

 

Occupational Exposure:                    Category I = Job involves tasks or procedures that have exposure to blood and potentially infectious materials during the performance of duties.

 





Skills/Requirements

ESSENTIAL RESPONSIBILITIES

  1. Demonstrates knowledge and expertise in the use of the Diagnostic Statistical Manual DSM IVR (and V, when legally vested), established standards of professional and ethical practice, and hospital and departmental policies and procedures.
  2. Prepares psychosocial assessment and diagnostic formulation -FHED assessment completed prior to admission; MHU addendum within 48 hours after admission- for use in assessment, treatment planning, discharge planning, and Level of Care (LOC) considerations.
  3. Demonstrates expertise in The Florida Mental Health Act -FS 394 (“The Baker Act”), possesses a thorough understanding of its application, and ensure compliance with all regulatory requirements.  Completes annual Baker Act Training.
  4. Demonstrates an advanced level of psychosocial and addiction/recovery assessments and intervention services, including individual, group and family therapies.
  5. Evaluates and responds to any urgent crisis of patient and/or their families.  Assess needs for care, provide direct crisis counseling and take immediate steps to stabilize clients in acute distress
  6. FHED- Provides consultation to attending psychiatrists and Emergency Department physicians within the scope of their practice and expertise in areas including mental health, addiction  and suicide assessments, trauma informed care,  crisis intervention and disposition planning;  MHU- Consult and confer with physicians and  nurses regarding all psych-social aspects of patient care including interventions, discharge planning and treatment referrals.
  7. FHED- Facilitates referrals needed, i.e.: admission to the Mental Health Unit, referral for detoxification services and all other follow-up care.  Make contact with referral source and provide any needed information to ensure the best outcome for the patient; MHU-. Refers patients to appropriate services with complete instructions for follow up care, and makes contact with referral source to provide relevant clinical information to ensure the best outcome for the patient.
  8. FHED- Completes utilization reviews by providing all relevant clinical information to insurance companies for any pre-certification of hospitalization; MHU- Assists Psychiatric UR staff with pre-cert and continued stay utilization reviews as needed.
  9. Compiles a referral source database, including ALF, nursing homes, substance abuse facilities, homeless shelters, etc. in order to provide the least restrictive, appropriate placement. 
  10. Provides patients and families with education, comfort, and relevant information in accordance with their care as needed.
  11. Maintains all records and documentation according to Hospital policy and procedure.
  12. Completes Special Projects and other job related duties as assigned by the Behavioral Health Unit Director or Medical Director.
