 Director of Nursing – RN Home Health
 
Job Description

The Director of Nursing assumes responsibility and accountability for assessing, planning, implementing, and evaluating care of home health patients in their home setting. Utilizing physicians' orders and professional skills the Home Health DON, in conjunction with the Medical Director and Home Health Agency team, develops, implements, and monitors a home health plan that meets each patient's specific needs. The DON participates in quality improvement (QAPI) activities, infection control monitoring, professional advisory committee activities, and Agency leadership. The DON directs and schedules case conference meetings. The DON is available at all times during the operating hours of the Agency and participates in all activities relevant to the  provision of home health services, which includes direct patient care.
 
Requirements:

•         Current registered nurse license in the state of Utah
•         Current Healthcare Provider CPR Certification
•         Two year RN experience, required
•         Director of Nursing experience
•         Home Health experience, preferred
•         Access to reliable and insured transportation
•         Lift and transfer patients independently
