Job Description

A rewarding full time job opportunity as a Community Resource Specialist at Access Community Health Centers (Access). The main function of the Community Resource Specialist is to provide case management to at-risk patients and coordinate necessary specialty referrals. This position is also responsible for tracking and reporting activities related to case management and referrals. In addition, this role will maximize the financial benefits to Access and its patients. This position assists patients by pursuing their enrollment in government programs such as Medical Assistance and insurance on the Marketplace through the Affordable Care Act. It also assists under-insured patients by administering Access’s sliding discount program.

Location:  Will work at one of our Madison, WI Access Clinics:  William T. Evjue Clinic, 3434 E. Washington Avenue OR Joyce & Marshall Erdman Clinic, 2202 South Park Street

Hours:  Monday thru Friday, 9 a.m. to 5 p.m.
 





Skills/Requirements

Required Qualifications:
1. A minimum of a High School Diploma or equivalent AND health care experience and/or social services experience OR Associates Degree in health care, social services or related field.
2. Basic computer software skills with programs such as Microsoft Word and Excel, as well as electronic mail and internet applications. 
3. Must obtain and maintain the status of a Certified Application Counselor by passing the applicable state and federal tests annually.

Preferred Qualifications:
1. Fluency in Spanish and English.
2. Experience in case management.
3. Experience in health benefits counseling.
4. Experience with diverse cultures.
5. Experience with medical software applications
6. Basic Microsoft software skills with programs: PowerPoint and Access
