Job Description:                                 PHARMACIST - SENIOR

 

Department:                                        Pharmacy

 

Responsible To:                                   Director of Pharmacy

 

Education/Training:                            Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) or graduate of a foreign college of pharmacy.  BLS required. Full time staff only: ACLS required within six months of start date.

 

Experience:                                           Five years previous experience in hospital pharmacy.

 

Skills & Abilities:                  Good physical and mental health.  Must be willing to work with the realization that errors may have serious consequences to patients.  Must have a memory for details and be alert to detect errors in compounding.  Must be able to cooperate with other employees and have the ability to supervise subordinate workers.  Must have ability to utilize all necessary reference book and textbooks related to medicine and pharmacy.  Ability to work independently with minimal direct supervision.  Ability to work cooperatively with health-system and pharmacy staff.  Ability to handle frequent interruptions and adapt to changes in workload and work schedule.  Ability to set priorities, make critical decisions, and respond quickly to emergency requests.  Ability to exercise sound professional judgment.  Ability to communicate effectively (orally and in writing).  Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, chemically dependent, rehabilitation, and skilled-nursing patients.

 

Physical Requirements:                      Repetitive use of hands and fingers (e.g. preparing IV admixtures, use of a computer keyboard).  May require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, unit dose-cassettes, and IV solutions and stooping or kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers).  Sitting, walking, or standing for long periods of time (4-8 hours) are often necessary.  Must be able to physically operate the equipment used for the job.

 

Environmental Conditions:               Potential for exposure to infectious patients or materials.  Potential for exposure to hazardous and toxic substances (including chemotherapy, cytotoxic drugs, and cleaning solutions).  Potential for sticks or cuts by needles and other sharp items.  Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

 

Occupational Exposure:                    Category III = Job involves tasks or procedures that have exposure to blood and other potentially infectious materials during the performance of duties.

              





Skills/Requirements

 

ESSENTIAL RESPONSIBILITIES:

  1. Accurately interprets, profiles and fills physicians’ orders.  Ensures all medications filled, checked and delivered in a timely manner.  Monitors drug therapy for duplicates, contraindications, interactions, incompatibilities, allergies, and dosing errors.  Documents interventions as needed.
  2. Review and monitors drug therapy that requires dosage adjustment per physician order.  Intervenes with physician to clarify medication orders, dosage, frequency, route as needed.  Documents changes in Medical Record as per telephone order.
  3. Aseptically prepares intravenous fluids, irrigation solutions, additives, chemotherapy and other sterile products.  Maintains appropriate documentation. Dispenses control drugs and maintains accurate records and inventory as required by law and pharmacy policies.
  4. Maintains inventory of pharmaceuticals and supplies, and ensures the proper storage condition, removing all medications past their expiration date and/or no longer suitable to be in inventory. Participates in stock control programs (ordering, inventorying, and monitoring usage) as required.
  5. Obtains emergency drug items or out of stock items as needed per pharmacy policy and reviews all non-formulary requests for formulary alternatives, prior to dispensing.  Ensures correct medications are filled, dispensed and delivered appropriately.  Directs and checks all activities of Technician admixing, packaging and labeling.
  6. Provides drug information to the hospital’s professional staff.  Provides pharmaceutical care to all ages of patients from newborns to the elderly.
  7. Participates in training new staff pharmacists, technicians or volunteers as needed.  Participates in routine back-ups or downtime procedures of the computer programs, drug information programs and databases.  Participates in quality assurance programs.
  8. Defines problems, collect data, establish facts and draw valid conclusions, interpret technical instructions, and deal with abstract variables.
  9. Performs other job related duties as assigned by Director.
