Job Description

Rainbow Hospice and Palliative Care, the region’s fastest growing network of palliative care providers, seeks board certified Advanced Practice Nurses for inpatient, ambulatory and community-based careers. 

As an integral team member, practice side-by-side with your patient’s primary physician, specialists and our own multidisciplinary teams.  Assist with complex symptom management, patient and family education, advance care planning and coordination between care delivery sites.  

Join our skilled and experienced clinical staff, nationally-recognized progressive leadership and a productive preferred-provider relationship with the state’s largest Catholic health system including 11 hospitals, 25 long-term-care facilities and their surrounding communities.  We are creating opportunities throughout Chicagoland including the far western and southern suburbs.  Qualifications include:

  • 2-3 years of clinical nursing experience in hospital-based or palliative care services preferred
  • Excellent communication skills and ability to work in a collaborative environment required
  • Current RN and APN licenses, as well as controlled substance and federal DEA license
  • Field and community-based practitioners are required to have a valid, unrestricted driver’s license
