Job Description

We are currently seeking qualified candidates to join our after hours triage team as a PRN Night and Weekend Supervisor at our Inpatient Hospice facility in Park Ridge. The candidate will provide skilled professional nursing care and case management during evenings and weekends for the patient/family/caregiver as prescribed by the physician and in compliance with Federal and State laws and regulations, the Illinois Nurse Practice Act, RHPC policies and procedures and CHAP standards of care.

Shifts covered include 4:30pm-12:00am on weekdays and 12 hour shifts on weekends (8a-8p or 8p-8a).

Skills/Requirements

A. Graduate of an accredited school of professional nursing required, BSN preferred.

B. Currently licensed as a Registered Nurse through the Illinois State Board of Nursing.

C. CHPN within the first two years of hire.

D. Three years of acute care nursing or 5 years of home care/long term care nursing required.

E. Maintains auto insurance and valid driver’s license in accordance with agency standards.

F. Possesses and maintains current CPR certification.

G. Good oral/written communication skills.

H. Is self-directed with the ability to work with little supervision.
